Day 63
Today is the day that I will continue to do long arc quads, 3-way hip by the step, and hip ABD & ER.  It is time to wake those muscles up.  Pain meds only if necessary.  I haven't taken more than 3-4 x 1/2 pill over the last 4 weeks.
Pain is minimal but there is a burning in the buttock and posterior lateral thigh.  Go figure, I had a major hip surgery. Still not able to ascend steps without cane unless I want some discomfort and not a stable feeling.  Bone grows better with weight to elicit production so...
I will come back as the day progresses to update on my attempt to activate cut or unused muscles
Well I did everything I thought I would and most of the exercises provided no lasting discomfort except...SLR with straight or bent knee and ABD against gravity.  There is nothing there besides pain.  Oh well it will take time and you have to test the waters
I am not however using the transfer bench to take showers but I am still getting help putting on and taking off socks and jeans

Day 61 & 62
I tried driving 2 hrs and the leg was getting uncomfortable even though I was on a cushion, the wheel was up at it's highest, the seat down and tilted back. An unexpected hard part was using cruise control and lifting the leg to engage the break.  Straight leg raises still suck. I still can't walk without a significant Trendellenberg gait ( penguin waddle ).  I saw myself in a mirror.  Still practicing going up/down steps with cane.
The discomfort is primarily behind the greater trochanter and the buttock
Tomorrow I plan to try side lying ABD.  The other day I did bilateral standing hip 3 way ( flexion, extension, ABD ) using the step height as a platform to touch.
With the cane I can walk decently until the tip catches as I get out of sequence but that happens less.  I can't wait , but obviously will , to see what I can do in 5-6 months.

Days 59 and 60
I have been attempting to walk with the cane more and increase tolerance to standing.  I was on my feet a few hours in a row yesterday and my buttock was hurting enough to necessitate taking 1/2 pain pill.  It still hurts this morning as does the pain from lifting the leg against gravity.
I need to start trying to strengthen the hip ER as they were cut through during surgery.
All this may be par for the course and may not be unexpected or unusual for this surgery rehabilitation.
I am still driving short distances and am still using the cushion to sit.  I am trying to go up steps 1 after the other using the cane.  I can not do it with out yet.
My limitations are less but the residual ones are harder to abolish and are taking more work
My work situation is concerning me as there is no way I could return to what I was doing at this point but that is in the future and I just have to deal with today doing the best that I can.

Day 58
Last night before going to bed I decided to try and do straight leg raises in the recliner.  I have been getting stronger it would seem so why not.  Well...it was not a good idea.  Both last night and this morning my thigh is hurting.  Once it subsides I hope to try in standing to take the flexion pressure off the hip. I tried very small bridges also and this too was not comfortable however I don't think there is any residual discomfort from them today.
I have again tried Trek poles instead of the cane but in the house is no good assessment because there isn't enough of a straight walk to assess them for this.

Day 57
Today I continued with walking with a cane. I'm not able to closely resemble normal walking without it yet.  Drove a very short distance again today.  I am finding that I can go upstairs one after the other as long as I put the cane on the step I'm stepping up onto and use my Upper body t o push down on the cane .
I walked up 9 steps several times today using this technique
My buttock is sore and I am considering taking 1/2 pain pill today after my escapades but I will give it 1/2 hour to see.

Day 55
Going down steps using a cane and railing is now easier and feels safer using step over progression or not 1 step at a time.  That is another improvement.
I tried to sleep on my right side with pillows between my knees but the didn't last long.  I had 2 pillows b/t my knees and layed on my non-surgical side to spoon with my fiancée nd that is great as long as I don't try to adjust my leg once in that position.  That temporarily  brings me back to memories of pain from weeks ago but as I said is short lived.
Fingers and thumb except for the little one remain with decreased sensation but with everything else that has been going on is a minor concern.  As things approximate " sort of normal " I'm sure I will be revisiting that with a hand specialist.

Day 54
I used the cane all day today.  I have problems sitting for any period of time.  My buttock (Isch tub ) burns but when I 1st rise the stiffness is very uncomfortable for a minute or two.  I tried to analyze my gait and I am not bending the knee and swinging the leg forward because it involves some rotation in the acetabulum which is uncomfortable.
Driving is still uncomfortable getting in & out and after sitting for a few minutes.  All things in time I guess.
My perspective of what, if, when, & how much with respect to work are realistically coming up.  All things in time as well.

Day 53
I decided to keep the Leiki cane after a trial of all the canes and shock absorbing trek poles in every combination I have.  The cane forces more weight onto mu leg and doesn't allow me to  lean to my left as much which in the long term is good for me.  I still can not ascend steps 1 after the other due to pain with pressure to push off the surgical limb.
I did spoon with my fiancée after putting 2 pillows between my legs.  That would be funny if she had to explain how she was getting pillow burns on her buttock as I was trying to position them correctly.  You can't set it up on your back and expect it to work right.
So I am doing more hip flexion and LAQ.  Yesterday I used 2 lbs and tomorrow I will go for 3.
With reality setting in I am turning my attention back to what will I be able to do in  4-6 weeks.  It is impossible for me to know especially since I have no idea how the osteotomy is healing.  There is still pain with sitting and leaning so time will tell to some degree.  I am not going to remotely jeopardize the healing of my hip no matter what
I did go out to get my haircut and to the Coumadin clinic to find out my blood  level is 5.??.  Guess no worries on it being too low anymore.  I have done nothing differently other than become more active and they did DC Rifampin but no changes happened 2-3 weeks later.  Maybe it has a long half-life or clearance time.
I have decided I want to win the lottery since my future is a bit uncertain and relaxed time rests the soul.

Day 52
Used a cane the entire day including walking for  out on the road.  The palm is sore.  I tried a little side walking and it didn't hurt at the moment but the lateral thigh is sore to the touch.
I was trying to compare glut by feel and it seems to me there is none on the right.  I can actually feel the hamstring all the way up to the butt bone.  Bridges are uncomfortable so I'm going to have to  try another exercise, perhaps prone hip extension.  I also tried going down the steps 1 leg after the other while holding onto a railing.  I could do this but there is no way I could do it without holding on.  So another exercise is to ascend/descend steps
My other Leiki cane came and for the amount of money it cost I don't feel it is worth it.  Shock absorption seems to help better than rigid but my trek poles aren't rady for me yet as I tried them.  Hmmm, another idea.

Day 52
Waiting on my cane to arrive.  I still haven't determined if I am going to keep it.  Leiki is a top brand but I am not sure how long I will be using this.  $74 is a lot right now with no income coming in.  I tried driving again and getting in/out of the drivers side is difficult still.  I thin my leg is getting stronger.  My knee does not go into hyperextension as much.  I think it is feeling stronger.  I believe I will try doing the exercises as I did yesterday even though it hurt some afterwards.  The long term benefit may be worth it.
More coming.

Day 51
Waiting for my new Leiki hiking cane without shock absorption to arrive.  I've gone back and forth on this but I am unable to progress WB with trek poles or without AD.
Sleep is much better so now it is time to slowly strengthen my hip ABD, flexors, and extensors as tolerated as we therapists like to say.
I will add more as the day progresses..
OK so I did 350 revolutions on the restorator, 15 LAQ with 2 pounds then standing gentle hip flexion 2 x 10, hip ext 2 x 10, & very gentle hip ABD 2 x 10. In addition I did UE supported mini-squats
Now my back and butt are uncomfortable enough I am considering  pain meds and I got a little spooked because I thought I was feeling the same calf sensations that led to the clot .  I know what to watch for so will keep an eye on it.
All things in moderation.

Day 50
Woke up a little sore today in the posterior aspect of the hip.  Today I am going to walk, use the restorator, do LAQ with 2-3 pounds, and practice sit to stand straight ahead instead of leaning with AD if needed.  I will repost afterwards to see how it turns out.
I have walked 8 blocks without incident.  I felt it a little on the inner thigh when I was going up an incline but this was a different sensation.  My back is getting a bit "tired' from staying flexed.
I was up on my feet about 30-40% of the time today and tried to drive to the corner store.  I might wait to do that again.  It was difficult getting in the left side because I am not used to ABD that leg so had to scoot across to the passenger side by backing into then swing the leg over with the seat all the way back, tilted back, and steering wheel all the way up.
I tried pushing a grocery cart for 4 small bottles of a vitamin water and found that in order to take the weight off the leg I had to push through my forearms since leaning on it while it was rolling was not pleasant.
Day 49
Tested various AD such as track poles etc to see if I could get by without ordering hiking cane.  I came back to the conclusion that I had to have a cane.  Went walking twice using 1 crutch and even though I thought I was bearing wt equally the left side of my back let me know I was leaning a lot on the opposite side. I ordered a hiking cane that does not have shock absorption even though that was available.
I am no longer having home health nursing and am transferred to the local Coumadin clinic.
I used the restorator 300 revolutions with minimal resistance.  Rotation of the hip still causes sharp pain.

Day 48
I took 1/2 of 1 pain pill last night as my but bone or ischial tuberosity was aggravated.  Today I am noticing what I can and can't do.  I can put shorts on the floor and in sitting work my surgical leg through the opening and lift my leg so that I can pull them up.  I can swing my leg up onto the bed without pain.  There is more ease in moving the leg out to the side. I am sitting on an elevated seat to eat dinners at the dining room table now
I can't bear full wt on my leg to go up a step pushing off the surgical leg.  I can't bear full wt on my leg to put on my other leg socks or anything.  I can't stand and bend at the waist to look into a closet, which I wasn't supposed to but forgot as I was looking for a cane.  @ hours later it is still bothering me in the posterior aspect of the hip joint.  If it continues for another 1/2 hour then 1/2 of 1 pain pill

Day 47
I didn't sleep well last night but that was an issue of medication.  I took a shower this morning using the transfer chair without incident.  I noticed the toenails on my right foot are beginning to resemble talons.  I need to ask someone or pay to get them cut
The last few days one of the discomforts is  n the area of the MTP 2-5 when I  push off on the right foot.  I imagine that is from using the forefoot from toe touch weight bearing .
A couple of other subtle changes noticed are the ability to lift the leg up onto the tub mantle or to slip the foot into the leg opening of shorts and lift up a little.  I can also swing the leg onto the bed with minimal assistance now.

 Day 46
I decided not to try and amb all day without AD as I was not presenting with an effective gait and it was hurting some.  So today I went back to using 1 crutch with a combination of rest and occasional amb in home without an AD.
My Coumadin level was 2.2 which is good
I am getting up from the recliner without an AD but am still leaning to the left.  That is the one area that I don't think I am remembering enough of is to not flex to pick things off the floor unless my leg is in extension.  I am still having lateral thigh burning with contact or pressure and Im not sure if that is from contact with superficial wires or not.
I was able to finish 0.8 CEUs necessary for CSCS recertification.  About time.

Day 45
I came down stairs without my crutch and went into the kitchen without an AD today.  Steps were very short and I waddled like a duck but there was no increased pain.
I expect tomorrow will be the last home health nursing visit as it should be.  Coumadin checks will be at the local clinic.  Since it is my right leg I am not comfortable with driving in the community.  If I had to push hard on the brake I suspect I would let off due to pain.

Day 44
Minimal discomfort today.  Took a shower using transfer bench.  Walked around the yard with 1 crutch but hit a soft spot and I think I rotated a bit but the pain was very temporary.  I an sitting at the dining room table for meals now using padded seat I got after surgery
I went to a festival and sat in a wheelchair the entire time.  I was afraid my hip would be very stiff as a result but it wasn't. Sleep is becoming better and is not as dreaded as it once was.

Day 43
Well I have started off the day in good shape.  There is some lateral hip discomfort but low level. .  I used the cane a lot during the day and went to a meeting without the transfer chair.  I found that I am not able to bear full weight and push off on the surgical leg.  I can go up 1 step at a time but doing 1 step over the other produced a deep pain.  Once last night as I was sitting on a sofa , which is relatively new, I moved and a bad pain popped up in the hip.  Luckily it only lasted a couple of minutes.
So life is returning to wards normal even though there is along way to go.  I  assume that if my coumadin level is normal then I will be shifting from home care to the local clinic to monitor my blood levels.
I still have to use the transfer bench for showers but that's ok.
The whole point of this endeavor is to show the progression from a painful, difficult surgery to a return to normal life and what to expect along the way.
I found that I didn't have the ability to stand as long without discomfort.  I am feeling a little more of a klunk with initial weight bearing.  After sitting I am having to take small steps.  The pain I experienced last night on the sofa, was repeated as I sat on the recliner.  I absentmindedly tried to hook my surgical heel in front of the other foot and the rotation produced a sharp pain in the postero lateral hip region.  Wow.
My Coumadin level remains 3.2 so is doing OK and they are preparing to shift my monitoring to the local clinic.
I will have to be wanded whenever I travel now but small price

Day 42
Today started off early as I wanted to get up for the 1st day of my fiancé's 14 y/o 1st day of high school.  That is the end of that.
I used a cane to go upstairs and around the house today & drove my car down the block;2 firsts. I rode the restorator 300 revolutions today My Coumadin was 3.1 and I don't have to inject my stomach tonight.  Yeah!!!
I took 1/2 pain pill tonight as the discomfort in my buttock has creeped up. I felt  things klunk a few times
Day 41
At the end of last night I had a discomfort that wouldn't go away in the area of the butt bone.  I woke up at 3AM not being able to get comfortable and woke up in a big sweat again.  It isn't all progress every day all day but that is life I guess or part of the process.
Still unable to do LAQ with any wts without pain yet so am trying 10 sec hold without weight.  Did do 200 revolutions on restorator in transfer chair
I still have numbness in the thumb through 4th digit so I have to decide how long to sit with this before getting  it checked out.  The causes are either CTS or cervical disc of which I have had both before.

Day 40
A pretty good day today so far.  I walked or crutched down the block and in the yard.  Today is the best it has felt in 3-4 days.  I may try the LAQ but only with 3 lbs today.  I went around on the restorator 200 times this morning after setting the seat on the transfer chair very high with cushions.  I may try cooking today sitting on a high seat
